Payment is required at the time of booking. Before proceeding, please review the Client Agreement and HIPAA Disclosure and Acknowledgment. These forms can be signed electronically for your convenience.

While the consultation fee is non-refundable, you may reschedule at no additional charge with at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice will incur a fee of 20% of the total booking cost.
